
The following information will help you understand the administrative requirements of enrolling your child and the operational policies that you need to know:
- You will be asked to complete an enrolment form containing information regarding your child’s health, development, custody arrangements, emergency contacts and immunisation status.
- Parents birthday for (Child care benefits - CCB)
- You need to name all persons who may deliver or collect your child. These persons must by 18 years and over.
- We require a copy of your child’s immunisation record prior to commencing.
- Completing Family background information form
- Health plan e.g. Asthma and Anaphylaxes management and any other allergies.
- Completing forms to register with centre link
- Complete forms for Direct Debit
Note: All information is strictly confidential. If any of your details on the enrolment form change you are asked to advise the Director or Administrator immediately. Information on enrolment and immunisation are routinely updated twice a year.
If we do not have the required days available, your child’s name will be placed on our waiting list. Preference is given to children already enrolled in our centre. Parents are request in fill out a form in November indicating care required for the New Year.
